Traveling PT - 13 week assignments with the possibility of renewal. Must be flexible on location as work location will vary.

Home Care PT - Must be willing to travel daily to various patient homes as well as SNF. Will be able to have permanent housing.

Doctorate of Physical Therapy (mandatory)

Current Foreign Physical Therapy License (mandatory)

Photocopy of U.S. Physical Therapy License (if you have one)

College Transcripts (mandatory)

Valid Passport

Birth Certificate (mandatory)

ICHP/FCCPT Visa Screen Certificate (mandatory)

English language test results letter – TOEFL iBT

Resume – Listing duties, awards, Pt. Diagnoses, Date resume created, contact info, 3 desired employment locations

Marriage Certificate. If either spouse was previously married you must provide a photocopy any divorce or death certificates.

Each child’s Birth Certificate, Passport and visa (if applicable)
PLEASE NOTE INTERNSHIP DOES NOT COUNT AS EXPERIENCE. Only list any experience you have obtained as a licensed PT on your resume.
